- January 4, 2013 – Emailed me and my mom’s “previous address” forms
- January 7, 2013 – Case worker emailed to reschedule Preliminary Interview for Jan 16, 2013.
- January 7, 2013 – Clearance paperwork emailed for me and my mom for states we’ve lived in the last 5 years
- January 16th, 2013 – Preliminary Interview with my local case worker
- January 21st, 2013 – Ordered two copies of my birth certificate, marriage, and Rob’s death certificate – ($157)
- January 26th, 2013 – FBI, state, gold seal – fingerprinting ($93)
- January 29th, 2013 – FBI, state, gold seal – Mom’s fingerprinting ($93)
- January 29th, 2013 – Submitted preliminary application to placement agency
- February 4th, 2013 – Submitted formal application to placement agency
- February 5th, 2013 – Mailed out 2 reference envelopes
- February 5th, 2013 – Mailed out health department inspection application ($90)
- February 8th, 2013 – Met friend to discuss in-person reference and being non-legal guardian
- February 11th, 2013 – Saw doctor #1 and had doc fill out form and sign
- February 11th, 2013 – Had physical done; extensive; lots of blood drawn
- February 12th, 2013 – Paid placement agency for application, online mgmt, and first agency fees ($3125)
- February 13th, 2013 – In-person reference mails in her letter
- February 13th, 2013 – Stopped by doctor for them to do a reading of a test I had done
- February 14th, 2013 – Paid placement agency for home study review ($250)
- February 14th, 2013 – Picked up slip from general doctor to go get another test at a lab.
- February 16th, 2013 – Coffee ceremony for homework assignment
- February 19th, 2013 – Watched two mandatory online training courses ($65)
- February 19th, 2013 – Ordered a complete driving record for me and mom ($24)
- February 25th, 2013 – Health Department inspection – passed!
- February 25th, 2013 – Received MVA records
- February 26th, 2013 – Mom takes Molly to get rabies vaccination
- February 28th, 2013 – Fire Marshall inspection – passed!
- February 28th, 2013 – Mom gets her physical exam
- February 28th, 2013 – Families (me) had conference call with placement agency re: their trip to Ethiopia and the children in the orphanages they visited
- February 28th, 2013 – Manager notarized Letter of Employment – dossier (made copy for home study)
- March 1st, 2013 – Full day of training scheduled
- March 8th, 2013 – Mom received physical test results – last piece for the home study
- March 10th, 2013 – Emailed case worker last documents for home study – officially 100% submitted!
- March 11th, 2013 – Stopped by my doctor to get a notarized health certificate – dossier ($15)
- March 12th, 2013 – PIP meeting – Pediatric dentist
- March 18th, 2013 – Received email from case worker about home study visit date
- March 24th, 2013 – Home study visit by case worker scheduled 9-11am
